The Arc of Davidson County and Greater Nashville – to create a short-term emergency fund to assist 170 disabled seniors in avoiding eviction and/or interruption of utility services.

Centerstone – to provide social support, outreach, and advocacy services to 120 older adults struggling with severe physical and mental illnesses and make them aware of the full spectrum of available treatment options.

Community Resource Center – for the purchase of quality of life items for over 1,000 seniors to live their lives with dignity; including personal care products, cleaning supplies, clothing, etc.

Council on Aging – to expand information resources and continue study and action on emerging issues important to over 600 seniors.

Cumberland University – to develop a mobile wellness center for over 1,000 senior citizens in order to improve and establish greater physical and mental health.

Domestic Violence Program – to provide enhanced services and better access to 100 elderly victims of domestic violence, sexual assault, and stalking and to provide a greater understanding of elder abuse in the community.

Faith Family Medical Clinic – to provide primary medical care for 384 seniors who would otherwise not have access to primary healthcare.

FiftyForward – funds Living at Home Services that provide services and funds for primary caregivers to allow over 2,000 frail seniors to remain in their homes.

Gilda’s Club – to provide funding for care services including workshops, lectures, group meetings, and evidence-based distress screening to 1,300 seniors.

Guardianship and Trusts Corporation – to support, increase, and enhance the provision of fiduciary services to elderly individuals who cannot afford it, including serving as trustee, conservator, or attorney.

Interfaith Dental Clinic – to provide funds for 500 seniors over the age of 65 to receive comprehensive and/or emergency dental care.

Martha O’Bryan Center – to provide funds for 1,400 seniors living in and around Cayce Place through the Emergency Food Bank, Family Support Team, and Meals on Wheels.

Mental Health America of Middle Tennessee (MHAMT) to raise awareness to caregivers and those concerned about memory loss through education, consultation, helpline, and print media including education series, caregiver support groups, caregiver conference, a guidebook, and information materials.

Music For Seniors – to provide funds for ongoing monthly live and interactive outreach performances for 1,200 seniors and to provide a free daytime Concert Series connecting area musicians with older adults.

The Nashville Food Project – to produce and deliver 6,000 nutritious, made from scratch meals to seniors; including gardening, food preparation, and transportation.

Nashville Public Television, Aging Matters – to fund two new documentaries of Aging Matters and follow-up discussions: “Aging in Place” and “Health and Aging”.

St. Luke’s Community House – to serve 140 low income seniors in west Nashville with case management support, mobile meals, emergency food boxes, commodities delivery, senior club social and recreational activities, mental health support, emergency financial assistance, and free tax preparation.

Second Harvest Food Bank of Middle Tennessee – to provide 28,800 Frozen Tray Pack Meals for 300 seniors, 3.600 Senior Nutrition Boxes for 300 seniors, Administrative/Overhead, and one electric scaled pallet jack

Senior Citizens Inc. dba FiftyForward – to provide programming and transportation for 5,000 seniors at the Bordeaux FiftyForward Center.

Siloam Family Health Center – to provide high quality, affordable healthcare to 400 uninsured and underserved seniors.

United Cerebral Palsy of Middle Tennessee – to provide basic safety and independent living support to 520 seniors by providing wheelchair ramps and essential medical equipment.

The West End Home Residents –  provided care for the residents who were under the guardianship of The West End Home Foundation.  The last resident passed away in April 2013.

Alive Hospice  – provides grief support for seniors and one-time assistance to replace four of the thirty beds within the residence.

Bethlehem Center – funds the Meals on Wheels program for elderly, disabled, homebound people and Active Seniors program for those who are still active but have limited opportunities for social interaction.  Seniors in both components live at or below the poverty line in the North Nashville area.

Council on Aging – increase programs and services that assist seniors and caregivers through presentations and legal seminars, information and assistance, and printed material on nutrition, wellness and  exercise.

Faith Family Medical Clinic – provides assistance for primary medical care for seniors, age fifty and over, who would otherwise be unable to access primary healthcare.

FiftyForward – funds Living at Home Services that will allow seniors to receive nutritious meals, participate in Adult Day Services, receive care management services, access services for those who have been victimized, and provide services for caretakers.

Gilda’s Club – provide the emotional and social support needed for seniors impacted by a cancer diagnosis.

Interfaith Dental Clinic – provide affordable dental care for low-income seniors; a portion will be used for building repairs to preserve the quality of services.

Legal Aid Society – funds provide for revision, translation and printing of two community education brochures:  “Paying for nursing home or home and community based care with the CHOICES program” and “Advance Care Plan and Health Care Agent”.  In addition, produce and translate a new brochure: “Health Care Surrogates and Conservatorships”.  Program includes six presentations made to seniors in Clarksville, Gallatin, Murfreesboro and Nashville offices.

Martha O’Bryan Center – provides funds for Emergency Food and Crisis Services Programs for seniors in Cayce Place and surrounding East Nashville community.

Matthew25, Inc. – funds personal case management for men ages 55 through 85 for substance abuse and/or mental health disorders.

Mental Health America of Middle Tennessee (MHAMT) – provides Aging and Alzheimer’s Services for Older Adults to address needs of senior caregivers through 4-week educational program, one-day educational events, monthly network meetings, and an aging help hotline.

Nashville Public Television, Aging Matters – funds the second year of a multi-year initiative designed to open a community wide dialogue for senior citizens in order to improve the quality of life.  The project will produce two new documentaries, televised panel discussions, 26 new Aging Matters spots, a website with streaming video of all documentaries, 12 community engagement events, and local distribution of up to 1,000 DVDs per year.

Progress, Inc. – provides safe, reliable transportation for seniors and when needed provides individuals to accompany patients to appointments.

Rebuilding Together Nashville – provides support for the “Building a Healthy Neighborhood” event.  “Building a Healthy Neighborhood is a one-time, three-day, special event on May 29, 30, and 31, focusing on the Cleveland Park and McFerrin Park neighborhoods, repairing homes and making modifications to keep seniors safe in their home.

Room in the Inn – provides at least 12 highly vulnerable, homeless women intensive case management for a one-year program including psychotherapy and recovery classes.  Also the program connects these women with long-term disability benefits, provides housing readiness, transition into housing, first month’s housing costs, and referrals to partner agencies.

St. Luke’s Community House – supports ongoing services to West Nashville seniors through case management support, mobile meals, senior club activities, mental health support, physical health support, transportation, emergency financial assistance and commodities delivery.

Second Harvest Food Bank of Middle Tennessee – provides 10 large-scale, 8,000-10,000 pound distributions of perishable and non-perishable food items to four Middle Tennessee Senior Centers.  Each distribution will serve approximately 200 senior households.

Siloam Family Health Center – provides high-quality, affordable health care to uninsured and underserved senior citizens.  Patients pay on sliding scale and at least 600 senior patient encounters will be funded.

Tennessee Respite Coalition – expands the Family Directed Respite Program.  Serves  50 senior caregivers through a voucher program.

The Alzheimer’s Association – provides early stage social engagement activities for seniors during the early stages of Alzheimer’s in partnership with The Frist Center for Visual Arts, Fifty Forward’s Music for Seniors and St. Claire Senior Center and Mindful Adult Day Center in Murfreesboro, TN.

Westminster Home Connection – provides funds to help repair, modify and maintain homes for 40-60 elderly residents in Davidson and surrounding counties.  Also provide moving services to aid elderly people.

Vanderbilt University Medical Center – funds established a career faculty development award at Vanderbilt University Medical Center, which will provide rotating support to leading faculty members in geriatric medicine. The award will exist in perpetuity and is intended to serve as a springboard for innovative research, positioning faculty members to apply for additional federal awards. Over the years, multiple faculty members will receive support from this award, thereby creating a cohort of former West End Home Foundation Scholars—a group of faculty members with rare scholarship in the field of geriatric medicine.

Family Faith Medical Clinic – provides medical exams for senior citizens who would otherwise not be able to access affordable primary healthcare.

FiftyForward – Living at Home Services assists frail or homebound seniors to maintain an optimum functioning level and live independently, including adult day services, meals on wheels, in-home care management and services for crime victims.

Gilda’s Club of Nashville – supports the development and implementation of program activities to address the emotional and social effects of a cancer diagnosis.

Interfaith Dental Clinic – helps meet the comprehensive dental needs of low-income senior citizens through the Comprehensive Care Program, Emergency Program and Smile Rescue Program.

Light in the Nations – trains Volunteer “care partners” who have contact with neighbors to meet immediate concrete needs, fight isolation, monitor individuals’ well-being, and provide fellowship and education.

Mental Health America of Middle Tennessee – provides education, skill building and respite care for caregivers of individuals with Alzheimer’s disease.

Metropolitan Nashville Teachers Apartment, Inc. – provides comprehensive safety system for residents required by Metro Nashville Government.

Nashville Public Television – presents the Senior Health, Wellness and End of Life Project,  a multiyear project highlighting the needs of senior citizens, including 8 documentaries, followup programs, senior health spots, website and community engagement events.

Second Harvest Food Bank of Middle Tennessee – provides food-insecure seniors with large supplies of groceries over the calendar year accompanied with educational and nutritional information.

Siloam Family Health Center – provides high quality, affordable health care to uninsured and underserved senior citizens.

St. Luke’s Community House – supports all aspects of established senior services program, designed to help seniors maintain independence and access services to support their needs.

Westminster Home Connection – provides materials and skilled labor to repair or modify homes for elderly who lack the means to maintain their homes in a safe and functional condition so they can remain in their homes.

YWCA of Nashville and Middle Tennessee – expands Domestic Violence Outreach Services program to address elder abuse by increased awareness and increased intervention.

Vanderbilt University Medical Center – evaluates the impact of two cognitive stimulation programs at a local assisted living facility to determine the potential impact of such programs; currently such benefits are unknown.

Centerstone of Tennessee, Inc. – funds for the Older Adult Service Advocacy Program that offers outreach and case management services to Davidson county older adults (50+) coping with a life crisis, bereavement, or neglect; many of the clients suffer from undiagnosed mental illness.

Council on Aging – support for presentations, research and production of materials on caregiving, grandparents raising children, end of life issues, help with legal concerns, training for volunteer drivers.

Faith Family Medical Clinic – funds to provide medical examinations for 238 senior citizens who would otherwise not be able to access primary healthcare.

Senior Citizens, Inc. – FiftyForward will reduce isolation for older adults who can no longer drive, have cognitive or physical disabilities and lack social outlets by providing Adult Day Services and transportation subsidies.

Bethlehem Centers of Nashville – leverage funds received for Seniors’ Circle, providing structured activities three days per week to improve the physical health and strengthen the emotional and spiritual health of 25 low-income, North Nashville elderly; also expand The Hot Lunch Program from serving 103 to 150 homebound and elderly in North Nashville.

Council on Aging of Greater Nashville – expansion of caregiver project, outreach on end of life issues, continuation of Face of Aging 2030, development of our newest community initiative,  ReNEW (nutrition, exercise and wellness).

Faith Family Medical Clinic – assist in providing primary medical care for seniors, age 55 and over, who would otherwise not have access to primary healthcare.

FiftyForward – Living at Home Services will integrate evidenced based practices (nutrition, depression screening, physical exercise and fall reduction/in-home safety risk assessment) into Adult Day Services, Meals on Wheels, care management and Respite Caregivers/Senior Companion programs.

Interfaith Dental Clinic – assist in meeting the dental needs of 185 low-income senior citizens during July 2010-June 2011 fiscal year through the clinic’s Comprehensive Care Program and Emergency Care Program.

Jewish Family Service of Nashville and Middle Tennessee – funds for the Emergency Preparedness Program that will offer materials and present a Personal Emergency Preparedness Program printed guide to assist local seniors establish a personalized plan for their use during emergency or disaster times, thus preventing or lessening of social disruption.

Light in the Nations – meet some of the immediate needs of seniors in low-income area of West Nashville known as The Nations; stay in regular contact with these seniors to fight isolation and monitor their well being.

Martha O’Bryan – support the health of seniors in Nashville’s most distressed community, the James A. Cayce Homes and surrounding east Nashville, funding portions of Meals on Wheels and Crisis Management programs focusing on senior’s physical and psychological well-being, while covering additional material costs to address food insecurity among elderly clients.

Metropolitan Nashville Teacher’s Apartment, Inc. – serve as a catalyst for fundraising to support a sprinkler and other fire and life safety systems mandated to be in place by Metro Nashville Government by September 30, 2012; the facility provides lower income seniors with an affordable facility in an optimal location for independent living.

Second Harvest Food Bank of Middle Tennessee – provide eight 1,200 pound distributions to each of three senior government housing facilities for seniors, serving 100 seniors each.

Shepherd’s Center of Madison – funds food program for seniors including holiday weekend meals and winter emergency packages.

St. Luke’s – support senior program designed to help residents of West Nashville maintain independence and access services to support their needs.

Centerstone Community Mental Health Centers – supports continued expansion of Older Adult Services (OAS) allowing a Registered Nurse to help older adults to be aware of and receive optimum mental and physical health services in Davidson County.

FiftyForward – provides funding for transportation services to/from two Senior Centers and Adult Day Care; assistance with transportation costs of Foster Grandparent, Senior Companion and Retired Senior Volunteer program volunteers; transportation costs for care managers to visit customers.

The West End Home Residents – provides care for the residents who were under the guardianship of The West End Home Foundation.

Guardianship and Trusts Corporation – establishes the West End Fund for the Elderly used to defray the costs of providing guardianship, conservatorship, attorney in fact, trustee and other necessary services to impoverished and disabled senior clients.

Senior Citizens, Inc. – expands Living at Home services (care management, in-home assistance, adult day care and meals on wheels) for frail seniors in South Nashville to help them live independently in their own homes.

Salvation Army Red Shield Family Initiative – provides intensive outreach and services to vulnerable older adults residing in East Nashville surrounding the Magness-Potter Center.
